We recommend MySQL network installation mode even for single-handed practice. Learn about the advantages of network mode. Single-computer mode is deprecated and should only be used for testing. This installation mode will be removed from future versions. Read more.

Installation in single-computer mode

  • In single-computer mode, databases are linked to a specific computer. Databases cannot be accessed from another computer.
  • You are still able to create multiple users.


  • FreeMedForms initiates the installation process and if the configuration file config-net.ini is not found in the user's resource hidden directory .freemedforms
  • .freemedforms directory is automatically created by default or can be specified on the command line via parameter --config. Read specific documentation.
  • Installation is quick and easy and does not require computer knowledge.
  • Do not forget to create a user other than the default FreeMedForms administrator to use in your day-to-day practice.
  • The installation procedure will create a specific folder on your local computer, where all your private data will be saved, depending on your platform:
    • Linux/Unix : /home/yourname/.freemedforms
    • MacOs : /Users/yourname/.freemedforms
    • Windows : %HOMEPATH%\.freemedforms
  • Please note that the folder's name, .freemedforms, begins by a point, which means it's a hidden folder. To make it appear in your favorite file manager, set it to display hidden folders.

  • The first step is to select the single computer mode (in the drop down box labeled “Single computer”)

  • This step is crucial and is reversible after installation.
  • You must select the central medical form that fits your practice. This file is the basis for all patients. You will be able to consult, add subforms that will only be active for the selected patient at the time of the addition.

Last step

  • Two options are available:
    • Clean databases already existing. The old database will be backed up. To recover, contact the development team
    • Create virtual data. A few medical records with sample data will be created. This option is useful for those wishing to try the software. It can also be used for testing by our beta testers. These data will not disrupt the normal use of the software because it is possible, thanks to preferences, to ignore them.
Click Done, FreeMedForms is now ready for use.

You are done

  • After a few seconds of work, FreeMedForms will display the login window.
  • Enter your user's credentials.
  • By default, FreeMedForms creates an administrator that can connect at any time with the following credentials:
    • Login: fmf_admin
    • Password: fmf_admin
  • It is strongly recommended to change the password of this administrator. Read the documentation.
